Currently our registration form is being updated, but please send an email to info@respiratorybooks.com with the following information:

What is your name?
What is the registration code included with your bundle?
Where did you purchase the bundle?
What date did you purchase the bundle?
You may be asked to produce documentation to support this purchase (a receipt) if not purchased via RespiratoryBooks.com. Note that your purchase has to have been less than 30 days ago.
